571. The role of an ambulatory Palliative Care Team PCT ; during exacerbations in the final stage of life of terminally ill cancer patients Christoph Wiese, Utz Bartels, Andrea VoenWellmann, Hannah Morgenthal, Michael Bautz, Margret Kriegler, Alexander Schultens, Friedemann Nauck, Bernhard Graf, Gerd-Gunnar Hanekop University Hospital Goettingen, Anaesthesiology, Gttingen, Germany Introduction: Terminal cancer patients suffer from many symptoms, e.g. dyspnea, pain. These can be shocking for next-of-kin causing helplessness, frequent calls for emergency medical services EMS ; and avoidable hospital readmissions thus making a peaceful death impossible. Although nearly 2.5% of all EMS tasks in Germany are calls to terminal cancer patients Palliative Care Teams PCT ; rarely get involved [1]. Aim of Study: To clarify whether Advanced Palliative Care APC ; can reduce use of EMS in those cancer patients. Methods: After informed consent 46 adults whose partner had died from cancer while in APC completed an anonymous survey. Results: In 19 cases 41% ; EMS had been called in the last six months of patient's life. Overall, 23 EMS visits were found 14x1, 4x2 and 1x3 visits ; , 16 led to hospital admission 69.6% ; . 21 calls 91.3% ; happened before vs. 2 during enrolment in APC. Other causes for EMS use were: dyspnea 39.1% ; , pain 26.1% ; , seizures 17.4% ; , unconsciousness 13% ; , and 1 fracture 4.4% ; . Conclusions: In their last 6 months 41% of patients used EMS at least once. 91% of these calls occured before a PCT took over, only 2 visits ensued subsequently. Like similar studies our data show that PCTs can help to significantly reduce use of EMS, number of hospital admissions, and stressful events for patients and relatives thus aiding a peaceful death [2]. | Continued from page 5 over assigned work. Several farmers mentioned that they kept a journal in the field, which they transferred to the next day's work plan. With a sigh, one farmer asked that the group remember to take time off to restore themselves, keeping the wax fresh on their surfboards literally and figuratively. To that end, Suzy Evans and Robbie Lee of Foundhorn Gardens announced that, after many years of vegetable production, they had decided to be generous with themselves and take a sabbatical from farming in 2007. Participants spoke of how honored they felt to live and work so close to the soil, with its power to produce safe and nutritious food for the world. These cultivators believe strongly in what they do, farming the land as its stewards. Their relationship with the land charges them with being responsible for its care until it may be passed along to the next generation. One event truly illustrated this point, and I had the good fortune to be there. shared stories about what farming the land and producing food meant to them. Farming was not just what they did to produce income, it is who they are, and what they embrace. Some farmers talked about their deep commitment to a life that can bring deep satisfaction and also despair, when a crop fails, a windstorm carries away a hoop house, or a flood innundates their chicken coop. The farmers discussed looking at the farm as a sustainable system. Some organic growers have gone into an inputbased system, moving toward a system that includes animal manures due to their belief that it provides a longer-lasting compost than one using strictly organic matter. Research has revealed a build up of potassium in the soil over time due to manure-based humus, and potassium increases have been found on Midwest on farms using animal manure in compost. The question of the hour was, "Can enough fertility be achieved with cover crops, so that animal manure is no longer necessary as a fertility input?" The farmers agreed that there is no easy yes-or-no answer due to the the entire input system. Italian farmers were mentioned as successful small-scale farmers by achieving the best soil fertility, without including animal manure in their farm fertilization systems. One farm reported not using dried henhouse chicken manure due to its nitrogen deficiency and, potentially, concentrated phosphorus Instead, they plan to convert to cover crops. A general rule of thought on the timing of compost manure applications, was that it is important to compost each year to provide optimum soil health, whether or not manure is used in the compost. Some farmers simply preferred not to use manure in their composting active systems. A few farms reported ongoing soil testing as a means to track fertility. The best soil tests track both soil chemistry and biology, .being equally important. Without soil biology present, the chemistry is undoubtedly compromised. Most agreed with this philosophy. Soil fertility is directly related to crop production results. Irrigation is also an integral part of the fertility formula. Without appropriate irrigation, plant uptake of calcium will be a problem. Although some fertilizer companies claim to improve the efficiency of irrigation, the farmers generally thought that the claim was not true. One farm reported that their soil test outcomes do not change over time, in spite of thousands of dollars spent on fertilizer inputs. Confidence continues to build in cover cropping. Progressively tracking soil test numbers with cover cropping practices can reveal randomness in testing. It was strongly suggested that farmers use the same soil lab. Using one lab over time reveals trends, if using the same sampling methodology. Farmers need some reward for all the composting and testing, aside from good crops. When organic matter goes up year after year, it is a reflection of good farming practices.

Figure 2. Cell-based model of coagulation. Reproduced with permission from the authours [Monroe et al 2002]. In this scheme coagulation occurs in three phases: initiation, amplification and propagation. In the initiation A ; FVIIa bound to TF activates FIX and FX. Factor Xa then activates FV on the TF-bearing cell, complexes with FVa and converts a small amount of FII to FIIa. In the amplification phase B ; , the small amount of initial FIIa activates platelets, causing release of -granulae contents including FV, activates FV, activates FXI and activates FVIII by cleaving it from vWf. Cofactors bind to the platelet surface before their respective enzymes. The FVIIa TF complex is shut down through the action of the TFPI in complex with FXa. In the propagation phase C ; , FXIa generated by FVIIa TF binds to the activated platelets and subsequently activates FX. This factor IXa is supplemented by FIXa generated on the platelet surface by factor XIa. FXa then moves directly into a protected complex with FVa resulting in a burst of thrombin generation and bronchial.

Radioactive isotopes are considered of greater use for diagnostic tests and labora tory investigations than for specific cancer therapy. 1131 is useful for studying the metabolism of iodine and functions of the thyroid gland, but only 10 to 20 per cent of thyroid cancers are amenable to isotope therapy. Surgery remains the preferred treatment for most nodular goiters and for some localized and non"iodine-absorbing tumors. For treatment of hyperthyroidism, the present trend is toward a single large exposure with very active material to avoid the effects of long-term radiation therapy. Use of repeated small doses for periodic functional evaluations increases the possibility of cell aberration. p32 is ad visable for polycythemia vera, particu larly in the late stages. Au'98 is promising for treatment of fairly well-localized can cer of the prostate gland. Special training in methods of local administration is needed, however. The most useful isotope is Co, a supervoltage therapeutic agent. For maneuverability, flexibility, and in tensity of output, Cot cannot be ap proached by other methods of equal power. Skin manifestations with super voltage therapy are slight and bumetanide.
Ild turkeys can be characterized as opportunistic feeders, meaning that they will eat a diversity of plant and animal species based on what is available during a particular time of year in the locale where they are living. Depending on the abundance or lack of a given food source, like acorns, turkeys may feed on alternative foods. Many wild turkey food habits studies have been conducted across the country. One study in Virginia illustrates the turkey's feeding habits. From examination of crops and gizzards from 537 turkeys in November and December, researchers found that turkeys had used 354 species of plants and 313 species of invertebrates. With the wide variety of plant species used by turkeys, how do you decide what to plant to enhance the habitat in your area? To help answer that question we sent a survey to the NWTF Technical Committee members asking for a list of the herbaceous, shrub and tree species they recommend planting in their state. Members of the Technical Committee are professional biologists, employed by the state fish and wildlife agency, and responsible for the turkey management program in their state. Their responses were combined by region to provide the recommendations in this publication. Their recommendations have been combined into three categories: those frequently mentioned in the survey, those more frequently mentioned, and those most frequently mentioned. Species are listed alphabetically within these categories, therefore, the order does not denote preference. Planting of the species listed will improve habitat not only for wild turkeys but many other types of wildlife as well. There are also other plant species not listed here that are used by wildlife. Hopefully this list of species will help you make the decision what to plant in your area. If you have additional management questions contact your state wildlife agency, Soil Conservation Service or county extension office. Most of the species mentioned in this publication are available through the National Wild Turkey Federation's Project HELP, Habitat Enhancement Land Program. The NWTF also has a publication, "Managing Openings For Wild Turkeys and Other Wildlife", that provides information on planting rates, planting times, soil conditions, ranges of various species, etc. 13-1.
