The bone marrows of patients with LON showed evidence of granulocyte maturation. Furthermore, filgrastim usually produced rapid granulocyte mobilization, suggesting that neutrophil egress rather than myeloid cell maturation is affected.12, 17 Recently, enzymatic cleavage of SDF-1 and CXCR4 has been proposed to mediate filgrastim-induced neutrophil and stem cell mobilization.19, 29, 30 Unfortunately, because it is not possible to measure bone marrow SDF-1 gradients in clinical samples, we cannot provide direct causative evidence for LON. While recognizing that plasma SDF-1 levels could reflect overall changes in bone marrow SDF-1 production, such as those associated with early B-cell lymphopoiesis, we believe it unlikely they could accurately reflect the complex and dynamic gradients that control neutrophil egress. Indeed, plasma SDF-1 levels are a composite of SDF-1 production, consumption, and clearance of the inactive molecule.22 Hence, we would not have expected to find a correlation between plasma SDF-1 levels and granulocyte changes. If, as we hypothesized, rapidly expanding B cells in the bone marrow transiently disrupts the SDF-1 gradients, one might expect to find a direct correlation with granulocyte decrements, which was observed.

"DBS is one of the most promising areas of OCD research because early studies show that it may help many within the approximately 20 percent of OCD patients for whom neither psychological nor drug therapy works, " said Dr. Suzanne Haber in a press release. Haber, the lead investigagor and a professor at the University of Rochester School of Medicine and Dentistry, went on to say, "Some patients have been able to venture out to work and school for the first time with DBS." DBS already has been used successfully to treat Parkinson's disease and epilepsy, and has been approved by the Food and Drug Administration FDA ; to treat these illnesses. The DBS system relies on a microstimulator to block abnormal nerve signals in the brain with electrical stimulation. Also known as a "brain pacemaker, " the microstimulator or "neurostimulator" ; is a small battery-operated device that is surgically implanted under the skin, usually near the collarbone. The microstimulator connects to the brain via an insulated wire that passes under the skin of the shoulder, neck, and head. This wire or extension ; connects to the lead or "electrode" ; , another insulated wire, implanted in the brain. A 2005 study showed that DBS may be an effective treatment for severe clinical depression. For mild grade 1 reactions, no treatment or treatment with topical anti-acne or anti-rosacea agents with anti-inflammatory properties can be started e.g. metronidazole gel or cream, erythromycin or clindamycin gel or lotion, benzoylperoxide gel or cream on the face or salicylic acid in alcoholic lotion on chest back ; . One should bear in mind that this topical treatment has drying properties and may aggravate xerosis. When the acneiform eruption is fading or becoming scaly, one should switch topical treatment to cream bases instead of alcoholic lotions or gels. In patients treated with erlotinib, no effect was seen with topical retinoids, vitamin D analogues or steroids [9]. Except in the presence of eczema see below ; , the use of topical or systemic steroids should be avoided since they are well known to induce or aggravate acne, acneiform eruption, rosacea or telangiectasia [28]. Moreover systemic steroids may interfere with the antibody-dependent cytotoxic action of EGFR antibodies. Although anticomedonal topicals such as retinoids tretinoin ; or a-hydroxyacids have been advocated in some reports [21, 61], we would not recommend them because of the lack of comedones [27] and the possible aggravation of xerosis and eczema [21]. For grade 2 reactions, topical treatment as for grade 1 can be used together with topical menthol cream or an oral antihistamine cetirizine, loratadine, hydroxyzine ; when itch is present and an oral tetracycline minocycline 100 mg day, lymecycline 300 mg day or doxycycline 100 mg day ; [21, 23]. Thanks to their anti-inflammatory properties, tetracyclines exert marked anti-acne effects, when given in a course of at least 3 months [28]. Oral isotretinoin is not recommended because of its overlapping side-effect profile xerosis, paronychia ; [43] and its unknown interaction with the antitumoural properties of EGFR inhibitors. In the case of grade 3 reactions, delay of therapy with EGFR inhibitors should be taken into consideration. In addition to the aforementioned topicals, compresses with physiologic solution can be applied in the case of acute and flu.
