Our data are consistent with the report by Paul and associates, 41 who indicated that FUdR initially inhibits DNA synthesis and is nonlethal, then later decreases the synthesis of nuclear RNA, nuclear protein, cytoplasmic RNA, and cytoplasmic protein, and eventually causes cell lysis. Myers found30 that the nucleotide FdUMP is an extremely potent inhibitor of the enzyme thymidylate synthetase. In our experiments, FdUMP, like FUdR, showed both cytostatic and cytotoxic effects. Addition of 1 M! 6.6 X 10"5 M of 3H-thymidine reversed the inhibitory effect of FdUMP at concentrations lower than 1 ug ml day 3. Among the four assays that were used, the 3H-thymidine uptake assay showed the lowest ID50 value in the case of FUR or FUMP, probably because this assay only measured those cells that actively synthesize DNA before division and was the most sensitive in detecting the effect of antimetabolites. The other two assays measured total cell numbers directly Coulter counter ; or indirectly by colorimetric assay hexosaminidase ; , including living and dead cells. The ID50 values that were determined by these two assays are consistently higher than the values determined by the 3 H-thymidine uptake assay. Because addition of 3Hthymidine reversed the inhibitory effects of FUdR or FdUMP on fibroblasts by day 3, we used 3H-adenosine uptake to measure the antiproliferative effect at the level of both DNA and RNA. The uptake of 3Hadenosine decreased as early as day 1 for all four fluorinated pyrimidines. In conclusion, we showed that FUR and FUMP are more potent than 5-FU, FUdR, and FdUMP, and have immediate inhibitory effects, probably because they do not require conversion to a more potent form. Because FUR showed the highest potency and is less expensive than FUMP.

Supports the HIAC route of delivery as superior compared with either systemic or portal venous if the aim of drug delivery is to enhance hepatic drug concentration and to deliver drug into measurable hepatic tumors.7 Physiologically, it is well recognized that most blood flow into the liver occurs via the portal vein 70%80% ; , whereas most oxygen-rich blood is delivered via the hepatic arteries, which constitute approximately 20% to 30% of blood flow. The challenge of which route of chemotherapy administration may be a beneficial method of drug delivery has been studied from a tumor vascularization perspective.8 Radiographically invisible hepatic metastases, otherwise termed micrometastatic disease, derive a blood supply from the portal vein.8 Administration of chemotherapy into the portal vein circulation has been reported in at least 10 studies; however, no conclusive evidence of decreased hepatic metastases or a survival advantage has been demonstrated by randomized trial meta-analysis for portal vein infusion therapy.9 In contradistinction to micrometastatic disease, measurable hepatic metastases are predominantly vascularized by the hepatic artery. 8 Delivery of chemotherapy into tumor is preferentially achieved by this route. Floxuridine FUDR ; , which is the primary agent used for HIAC, achieves a 400-fold increased intratumoral concentration after hepatic arterial delivery as compared with systemic infusion and a 15-fold increase compared with portal vein infusion.7, 10 Beyond enhanced delivery of the drug into the liver by arterial infusion, a further factor and key component of hepatic regional drug activity is the potential requirement for an agent i.e., 5-FU ; to be converted into an active metabolite FUDR ; . Hepatic extraction between these drugs is widely variable i.e., FUDR 5-FU ; , such that when HIAC infusion is used, FUDR therapy achieves higher hepatic concentrations and is almost completely first-pass extracted, with limited systemic spill, thus resulting in lessened side effects.10 These data support that the HIAC route of administration has value in achieving enhanced hepatic drug concentrations compared with systemic venous and portal vein delivery, that HIAC enables drug delivery into hepatic tumors based on the tumor vascular supply, and that different drugs have differential extraction within the liver once infused. In the next section, the significance of these observations within a clinical context will be discussed and fuzeon.

The monetary value of the unit collective dose has been assigned a value of 20.000 ECU manSv - 1 . The costs of the different foodstuff categories and of relocation are based on Danish economic conditions. These cost levels are, however, believed to be similar for the other Nordic countries, but this has to be verified in due course.
