There exist no substitutes for cocaine, and the compulsive craving it arouses often makes health workers feel as if they are having to fight it with their bare hands. Overdoses of injecting cocaine and the ensuing complications convulsions, heart attack ; are the main causes of drugrelated emergency hospitalisation in the United States. These cases are on the rise in Europe, although they are still unfamiliar to medical staff. Cocaine abuse also causes serious psychiatric disorders: hallucinations, paranoid states and violence. These consequences are all the more frequent when the context of drug and hydrea.
I honestly believe that most wounds fail to heal because of inappropriate assessment, and therefore inadequate treatment. I said `most.' It's not a hundred percent, because we know we do have those problematic wounds that just aren't kicking in. Those are the ones where I'm most inclined to start using these supplemental products, once we've debrided so at least we have a bed that's primed to heal. Ms. Brown-Etris: I was at a facility recently where the nurses typically obtain the order, "Use product X until wound healed." I'm on a mission in this facility to have the tag "until healed" not written. I had a conversation with one of the nurses and my question to her was, does this mean if the wound never heals we just keep using this dressing? When do you assess and recognize that what is being used is not effective any longer? Taking it a step further, I informed her that the dressing she just wrote "until healed" for is not designed to close the wound. It's designed to remove the necrotic tissue debris, absorb the exudate, and promote a nice granulation tissue base. A treatment change is then necessary. But the wrong order is now written, so nobody is going to think to assess the wound for the necessary treatment change. Dr. Mostow: That was a point that I was making. Assessment and re-assessment, and knowing where you're going. There have been some nice, new papers lately trying to address how fast is "healing fast enough." In fact, this is kind of the question and lead-in to what you were asking before: When do you use the Oasis dressing? With Oasis, you get a nice, relatively cheap product that at least in our early experience really seems to jumpstart the wound. For those wounds where you're making progress but even with debridement and offloading it's not moving fast enough, you ask yourself, "What can you do to speed it up?" That's one of the questions we're talking about answering in some of these studies. Can we find those people who, even if they're healing, are they going to heal faster than the ones who didn't get the Oasis? Preliminary studies suggest that it really is going to help speed things up. Ms. Brown-Etris: I think part of what you should do, if possible, is explain to your patients what you expect. I recently evaluated a patient in jail with large fasciotomy leg wounds. The young man had been shot. I said to him, "Here's what we're going to do." I explained the present tissue base to him, granulation tissue, muscle, etc. I explained my plan, what dressing would be used, and the expected healing response--why one side would heal quicker than the other. When I'm back in two weeks, here's what I expect to see. Then I will change to a dressing that will be on for X amount of time that will promote wound closure. My patients want to know the long-range plan. They have a road map that they can follow, and I'm looking ahead and guiding them through healing. I'm examining the wound and saying, "Here's what we have today, here's our plan, this is what you need to do to help me get to closure." Ms. Milne: The literature shows that you can predict healing in venous stasis ulcers but as far as pressure ulcers and surgical wounds, I don't think the literature is out there yet. It's probably a challenge to us wound care professionals to come up with outcome standards so we know when we're off track sooner. Dr. Niezgoda: I absolutely agree with you. I think that we can find some guidance in the literature for different wound categories. Certainly the patient wants to know when will it heal. If we can make an educated guess and give them a time framework, that is very helpful to the patient. We are asking them to do a lot. They want to know how long they are expected to submit to it. Dr. Mostow: I always tell patients: "The game plan is as follows. We might have to modify it, but here's what I envision right now." Then I describe my approach. Ms. Brown-Etris: That's right.
