Design: Antenatal and perinatal factors and early clinical, electroencephalogram EEG ; , and magnetic resonance imaging MRI ; findings were compared with neurodevelopmental outcome in 24 children with evidence of cerebral infarction on neonatal MRI. Results: Out of 24 infants, 19 had an infarction in the territory of a major cerebral vessel and 5 in the borderzone between cerebral arteries. Neuromotor outcome was normal in 17 and abnormal in 7 infants. Of these 7 infants, 5 infants showed a definite hemiplegia, whereas the other 2 showed some asymmetry of tone or function but no definite hemiplegia. None of the adverse antenatal or perinatal factors was significantly associated with abnormal outcome. Neonatal clinical examination was also not always predictive of the outcome. The extent of the lesion on MRI was a better predictor. In particular, it was the concomitant involvement of hemisphere, internal capsule, and basal ganglia that was always associated with an abnormal outcome, whereas the involvement of only one or two of the three tended to be associated with a normal outcome. EEG was also very helpful. Abnormal background activity either unilateral or bilateral was found in 6 infants, and 5 out of 6 developed hemiplegia. Evidence for implicating increased myocardial work in the production of the lesions under consideration is substantial. The procedures involved increase considerably sympathetic nervous system activity and or the work of the heart--struggling during prolonged forceful restraint, cold, injection of sympathomimetics, injection of large doses of vasopressin, etc.2 Severe myocardial stimulation by sympathomimetics has been shown to produce myocardia] lesions even in the hearts of otherwise normal animals, 22 and these lesions are markedly accentuated by DOC, but not by triamcinolone.21 It is of interest to note that steroid pretreatment, which causes a considerable decrease in myocardial potassium table 1 ; , promotes the production of lesions by epinephrine and norepinephrine, whereas cathartic-electrolyte treatment, which does not alter myocardial potassium significantly, does not.2 Increased myocardial work might accentuate the lesions at a given myoeardial potassium level or cause further potassium depletion. Heart and skeletal muscle are known to lose potassium during activity, 13' 2: i and with exertion to fatigue, this loss is of sufficient magnitude to make a major contribution to the depletion necessary to induce morphological changes. Exercise-induced loss can occur from tissues which are already potassium depleted, 23 and it is quite possible that even a small additional loss of potassium might precipitate the development of anatomical lesions in hearts with pre-existing borderline depletion. The reported beneficial effects of magnesium salts3' 24 also are entirely compatible with the assumption that the "electrolyte-steroid-eardiopathy" is due primarily to potassium depletion. Interaction of the two ions has been observed in several quite diverse situations. Of particular interest are the observations that during magnesium deprivation, skeletal muscle magnesium and potassium are depleted in parallel, 25 and that infusion of magnesium causes a rapid fall in plasma potassium levels, 20 the potassium presumably being shifted to an intraeellular locus. The immune response is complex in this setting, and includes allergic-like manifestations and circulating immune complexes.26 It is difficult to know whether clinical signs during the migration phase are directly due to schistosomula or rather to the immune response to schistosomula antigens. Eosinophils, with their potential cardiac, vascular, and pulmonary toxicity, also seem to play a major pathophysiologic role. Indeed, whether triggered by a parasite during invasive helminthiasis or due to a pure inflammatory disease such as the hypereosinophilic syndrome, hypereosinophilia is associated with strikingly similar cerebral complications.9 In this setting, corticosteroids seem more appropriate than praziquantel.
