Adrenergic blockers -- Adrenergic blockers are inhibited by amphetamines. Antidepressants, tricyclic -- Amphetamines may enhance the activity of tricyclic antidepressants or sympathomimetic agents; d-amphetamine with desipramine or protriptyline and possibly other tricyclics cause striking and sustained increases in the concentration of d-amphetamine in the brain; cardiovascular effects can be potentiated. MAO inhibitors -- MAOI antidepressants, as well as a metabolite of furazolidone, slow amphetamine metabolism. This slowing potentiates amphetamines, increasing their effect on the release of norepinephrine and other monoamines from adrenergic nerve endings; this can cause headaches and other signs of hypertensive crisis. A variety of toxic neurological effects and malignant hyperpyrexia can occur, sometimes with fatal results. Antihistamines -- Amphetamines may counteract the sedative effect of antihistamines. Antihypertensives -- Amphetamines may antagonize the hypotensive effects of antihypertensives. Chlorpromazine -- Chlorpromazine blocks dopamine and norepinephrine receptors, thus inhibiting the central stimulant effects of amphetamines and can be used to treat amphetamine poisoning. Ethosuximide -- Amphetamines may delay intestinal absorption of ethosuximide. Haloperidol -- Haloperidol blocks dopamine receptors, thus inhibiting the central stimulant effects of amphetamines. Lithium carbonate -- The anorectic and stimulatory effects of amphetamines may be inhibited by lithium carbonate. Meperidine -- Amphetamines potentiate the analgesic effect of meperidine. Methenamine therapy -- Urinary excretion of amphetamines is increased, and efficacy is reduced by acidifying agents used in methenamine therapy. Norepinephrine -- Amphetamines enhance the adrenergic effect of norepinephrine. Phenobarbital -- Amphetamines may delay intestinal absorption of phenobarbital; co-administration of phenobarbital may produce a synergistic anticonvulsant action. Phenytoin -- Amphetamines may delay intestinal absorption of phenytoin; co-administration of phenytoin may produce a synergistic anticonvulsant action. Propoxyphene -- In cases of propoxyphene overdosage, amphetamine CNS stimulation is potentiated and fatal convulsions can occur. Veratrum alkaloids -- Amphetamines inhibit the hypotensive effect of veratrum alkaloids.

In the U.S., the incidence of HIV AIDS is increasing most rapidly among African-American women. While the vast majority of cases occur in urban areas and nearby communities, the spread of HIV to and through rural areas is of increasing concern. Results of a recent study that looked at HIV risk in this group of women were published by Richard Crosby, PhD, and others, in the April 2002 issue of American Journal of Public Health. This study compared HIV-associated sexual health history, risk perceptions, and sexual risk behavior of low-income rural and non-rural African-American women who were attending federally funded Special Supplemental Nutrition Program for Women, Infants, and Children WIC ; clinics in Missouri. Participants were women receiving WIC benefits aged 18 years or older from 27 clinics in 23 counties who agreed to participate in an anonymous survey that focused on condom use. A total of 571 women 25% of women who filled.
Patients, providers, and insurers may be reluctant to cooperate with the effort to collect data. Patients may decline to volunteer such information if they fear it will lead to racial discrimination or will be used for medical or life ; insurance underwriting purposes. The fear of being sued for discrimination or "racial profiling" may prevent some hospitals, physicians, and insurers from collecting the data.100 Thus, any data collection effort must be explained in a way that makes it clear that any data collected will be used only for the purpose of improving the quality of health care.
Information for Patients: Amphetamines may impair the ability of the patient to engage in potentially hazardous activities such as operating machinery or vehicles; the patient should therefore be cautioned accordingly. Prescribers or other health professionals should inform patients, their families, and their caregivers about the benefits and risks associated with treatment with amphetamine or dextroamphetamine and should counsel them in its appropriate use. A patient Medication Guide is available for ADDERALL. The prescriber or health professional should instruct patients, their families, and their caregivers to read the Medication Guide and should assist them in understanding its contents. Patients should be given the opportunity to discuss the contents of the Medication Guide and to obtain answers to any questions they may have. The complete text of the Medication Guide is reprinted at the end of this document. Drug Interactions: Acidifying agents -Gastrointestinal acidifying agents guanethidine, reserpine, glutamic acid HCl, ascorbic acid, fruit juices, etc. ; lower absorption of amphetamines. Urinary acidifying agent - ammonium chloride, sodium acid phosphate, etc. ; increase the concentration of the ionized species of the amphetamine molecule, thereby increasing urinary excretion. Both groups of agents lower blood levels and efficacy of amphetamines. Adrenergic blocker -Adrenergic blockers are inhibited by amphetamines. Alkalinizing agents -Gastrointestinal alkalinizing agents sodium bicarbonate, etc. ; increase absorption of amphetamines. Coadministration of ADDERALL and gastrointestinal alkalizing agents, such as antacids, should be avoided. Urinary alkalinizing agents acetazolamide, some thiazides ; increase the concentration of the non-ionized species of the amphetamine molecule, thereby decreasing urinary excretion. Both groups of agents increase blood levels and therefore potentiate the actions of amphetamines. Antidepressants, tricyclic -Amphetamines may enhance the activity of tricyclic or sympathomimetic agents; d-amphetamine with desipramine or protriptyline and possibly other tricyclics cause striking and sustained increases in the concentration of d-amphetamine in the brain; cardiovascular effects can be potentiated. MAO inhibitors -MAOI antidepressants, as well as a metabolite of furazolidone, slow amphetamine metabolism. Ance of hazardous tasks, such as operating machinery or driving a motor vehicle. Should be used with caution in patients with a history of seizures and, because of its autonomic activity, in patients with a tendency to urinary retention or increased intraocular tension. Tachycardia and postural hypotension may occur more frequently than with other antidepressant drugs. Should be used with caution in elderly patients and patients with cardiovascular disorders; such patients should be observed closely because of the tendency of the drug to produce tachycardia, hypotension, arrhythmias, and prolongation of the conduction time. Myocardial infarction and stroke have occurred with drugs of this class. On rare occasions, hyperthyroid patients or those receiving thyroid medication may develop arrhythmias when this drug is given. Usage in Children: Not recommended for use in children because safety and effectiveness in the pediatric age group have not been established. Usage in Pregnancy: Safe use in pregnancy and lactation has not been established; therefore, use in pregnant women, nursing mothers, or women who may become pregnant requires that possible benefits be weighed against possible hazards to mother and child. Precautions: When protriptyline HCI is used to treat the depressive component of schizophrenia, psychotic symptoms may be aggravated; likewise, in manicdepressive psychosis, depressed patients may experience a shift toward the manic phase; paranoid delusions, with or without associated hostility, may be exaggerated. In any of these circumstances, it may be advisable to reduce the dose of protriptyline HOl or to use a major tranquilizing drug concurrently. Symptoms, such as anxiety or agitation, may be aggravated in overactive or agitated patients. When given with anticholinergic agents or sympathomimetic drugs. including epinephrine combined with local anesthetics, close supervision and careful adlustment of dosages are required. May enhance response to alcohol and effects of barbiturates and other CNS depressants. Possibility of suicide in depressed patients remains during treatment and until significant remission occurs, this type of patient should not have access to large quantities of the drug. Concurrent administration with electroshock therapy may increase hazards of therapy, such treatment should be limited to patients for whom it is essential. Discontinue drug several days before elective surgery, if possible. Both elevation and lowering of blood sugar levels have been reported.

Empirical antibiotic treatment of AECB has generally been accepted as standard practice due to the recognized inaccuracies of sputum culture and the time required before culture results are available. Accumulating data suggest that there seems to be a benefit associated with it, especially for more severely ill patients.4, 5 Acknowledging this, recent guidelines state that antibiotic treatment of AECB should be initiated if bacterial infection is suspected.1, 23 Since treatment is empirical, the activity of the drug against expected bacterial pathogens is a key factor and protriptyline.
